Course Description

Everyone makes mistakes. This is no less true of organizations than of individuals, but organizational failures can have far-reaching consequences. When do mistakes tend to occur? Why do entire organizations overlook foreseeable problems? What is the nature of our obligation to mitigate mistakes? This SEV will explore the these questions across a variety of context through readings related to famous (and not-so-famous) mistakes including the Challenger and Columbia accidents, the BP oil spill and others.
